Release Forms

Students are asked to sign a blanket media release, granting the School the right to use recordings, video, and photographs created during their study at the School of Music. The form contains complete details of this agreement.

•  Unlimited Media Release  

Guest artists, faculty, and student soloists for specific performances may use the Limited Media Release form.

•  Limited Media Release  

In order to stream student degree recitals and ensemble performances, please complete the approval form below.

•  Live Stream Authorization  

Please print, sign, and mail or fax the appropriate form to the Office of Concerts and Media.
